Can You Stop a Cavity Before It Starts?

You’re busy getting ready for your day, and of course you’re starting your day off right by looking after your dental health. A two-minute brushing, some careful flossing, and—wait, is that a white spot on your tooth?

If your mirror reveals a patch of white on your enamel, it could be a symptom of early tooth decay.  Does this mean a cavity is necessarily in your future? Perhaps not, if you act promptly to reverse the damage!

Cavities, after all, don’t suddenly appear in our mouths. Tooth decay is a process, and it begins with a weak area in the enamel that often appears as a white spot on the tooth. These weak spots are known as “incipient cavities,” because they can easily become cavities if left untreated.

Why do these weak spots occur? And how can we repair them? Let’s look at some tooth science.

  • Demineralization

Your tooth enamel is one of the marvels of the human body. Enamel is mostly made of calcium and phosphate ions, minerals which combine to form hydroxyapatite crystals. These crystals create a substance so strong that tooth enamel is the hardest part of the body—harder even than bones.

But it’s not indestructible! If you don’t care for your teeth with regular brushing and flossing, you’re leaving your enamel open to attack.

The bacteria in plaque convert the nutrients they get from the food we eat—especially sugars—into acids. These acids strip away the calcium and phosphate minerals which are the building blocks of our enamel, weakening the surface of the tooth.

Over time, these weak spots get bigger and deeper as bacteria and acids continue to attack the tooth surface. Eventually, cavities form, damaging the enamel surface, and, over time, even reaching into the dentin and the pulp inside the tooth. Once a cavity has formed, you need dental treatment to clean away the decay and repair the tooth structure.

The good news is that, when caught early, it’s possible to rebuild a weak spot in the tooth enamel before a cavity can fully develop.

  • Remineralization

While acids strip minerals from your enamel, your body is also replacing those lost minerals all throughout the day. How does this balancing act take place? With the help of saliva.

The very minerals which acids strip from your enamel are found in saliva. Saliva bathes your teeth with calcium and phosphate ions which, in turn, bond with your enamel to rebuild and restore enamel strength. When missing minerals are replaced like this, it’s called remineralization.

But this careful balance of demineralization/remineralization can become upset. If you neglect brushing and flossing, for example, plaque buildup will increase the amount of acids your teeth are exposed to. This means that your body won’t be able to replace the minerals as quickly as they are stripped away, and the result is a white patch on your tooth where the enamel has lost mineral strength.

Fluoride toothpastes and fluoride treatments are some of the most efficient ways to remineralize your teeth. Fluoride is attracted to the minerals in your enamel. It bonds with the tooth surface, and, once there, it helps attract the calcium and phosphate ions in your saliva to restore lost minerals and repair weak spots.

But fluoride does more than just restore your tooth’s strength—it improves it! Fluoride ions join with the calcium and phosphate minerals in your teeth to form fluorapatite crystals, which are even larger and stronger than hydroxyapatite crystals.

Early treatment is your best opportunity to reverse the early stages of tooth decay. Keeping up with your brushing and flossing, eating a dental-healthy diet, seeing Dr. Kerry Olszewski regularly, and treating incipient cavities promptly will help ensure there are no unwelcome surprises in the mirror—just beautiful, healthy smiles!

Periodontal Health during Pregnancy

Congratulations! Your pregnancy is a time of joy and excitement—and a time to take special care of yourself. You’ve discovered that pregnancy brings many physical changes, but it may still come as a surprise to learn that pregnancy hormones can affect your oral health as well. What should you look out for?

Gingivitis

Beginning in the second or third month of pregnancy, your gum tissue may show signs of gingivitis. Pregnancy hormones can cause an increase in the blood supply to your gums and affect the way your gums respond to plaque. These changes may lead to gums that are swollen, red and more likely to bleed upon brushing. This early form of gum disease should be treated as quickly as possible to avoid a more serious condition called periodontitis.

Periodontitis

Without treatment, the inflammation caused by gingivitis can increase. Periodontitis can lead to gums pulling away from the teeth, creating “pockets” that can be home to infection. These infections can lead to bone and tooth loss, so professional treatment is a must.

Pregnancy Granulomas

If you find a dark red swelling along the gumline or between two teeth, it might be a pregnancy granuloma. These granulomas are thought to be triggered by pregnancy hormones and may be a reaction to plaque or some other irritant. They often disappear once your baby is born and usually don’t cause any bother, but if you develop discomfort eating or speaking, your dentist might suggest removal.

You are looking for every way possible to provide your baby with the best start in life, so it is important to know that some studies have suggested a link between periodontal disease in pregnancy and complications such as pre-term delivery and low birth weight. Here are some important ways to maintain your oral health during pregnancy:

  • Call Dr. Kerry Olszewski when you find out you are pregnant. We have suggestions for your dental care that you can use immediately.
  • Keep to your regular schedule of dental examinations and cleanings at our Mill Creek office. If you find your gums beginning to show signs of gingivitis, call our office for an appointment. You might need to have your teeth cleaned more often during your pregnancy to avoid plaque buildup.
  • Maintain your daily dental hygiene. Be sure to carefully brush along the gumline to discourage plaque formation. If you have not switched to a soft bristle toothbrush, now is the time! Talk to us about possible rinses or other at-home treatments.
  • See a periodontist if needed for more serious gum problems.

Flossing Fixes

A length of floss plus your teeth is about as low-tech as it gets. But, as with so many other “simple” skills, it helps to learn just the right technique to avoid common mistakes and to make your flossing as effective as it can possibly be.

  • Choose the Right Floss for You

You’re getting ready to go out, and your floss keeps getting stuck, shredding, or snagging. You might try waxed flosses or flosses treated to glide easily through the teeth if this is a difficulty. (But do call us if it happens a lot—it could be a problem with a restoration, or a cavity, or some other condition we should address.) If you have the opposite problem, too wide a space between teeth for effective flossing, there are dental tape flosses that work with wider spacings. Braces? There are even specially designed dental flosses that thread between brackets and wires to access hard-to-reach plaque and food particles. If you’re unsure which product will work best for you, we have recommendations.

  • Don’t Be Too Hard on Yourself

Did you know dental floss is a handy kitchen tool? You can use (unflavored) floss to cut cakes into even layers, slice cheese, or divide a log of cookie dough into perfect rounds. Just pull the floss taut and saw away. But let’s not use this technique on delicate gum tissue! Gums can be injured by a vigorous, sawing motion. Instead, gently guide the floss between the teeth to the gums, and, when you reach the gum line, gently ease the floss up and down the tooth surface. But do remember, sometimes the gums are sore and sensitive because of too little flossing, not too much. Proper cleaning will help keep your gums both healthy and pain-free.

  • Technique Counts!

We often use floss to remove food particles from between the teeth, which provides instant dental gratification. But you are flossing for the long term as well. Proper flossing removes the plaque that leads to cavities from places your brush just can’t reach. Make sure you floss between each tooth, and don’t forget the back of those teeth on the end. The next time you visit our Mill Creek office for a cleaning, let us demonstrate the most effective techniques for gently removing plaque from beneath the gum area and on the tooth’s surface.

  • It’s All in the Timing

How much time should you spend flossing? That answer will depend on your individual needs. For some people, thorough and careful flossing once a day will be sufficient. For others, flossing more often might be advisable. We can help you decide how often and how long to floss.

It might take some time and practice to learn to floss effectively, but you will find your technique gets better and your flossing is accomplished more quickly once you have the basics down. If Dr. Kerry Olszewski can offer any suggestions, don’t hesitate to ask!

Let's Talk About Your Roots!

You’ve learned a lot from your dentist and your family about how to take care of your teeth. You brush and floss, you wear a mouthguard when you’re active, and you have regular checkups and cleanings at our Mill Creek office.

And everything you’re doing is wonderful! The smile you see each day in the mirror reflects your hard work.

But what about the part of your smile you can’t see? Let’s take a look beneath the surface, and talk about your roots.

While our teeth look solid from the outside, there’s really a lot going on inside each tooth.

  • The outside of every tooth is covered with a hard protective surface.

Enamel is the hardest material in our bodies, and covers the crown. This is the part of the tooth we can see above our gums.

Our roots are hidden below our gums, and are covered with cementum instead of enamel. Cementum is hard, but it’s not nearly as strong as enamel.

  • Just below a tooth’s enamel and cementum layers, we have dentin.

Dentin is a hard tissue, only a little softer than enamel. Most of our tooth is made of dentin. It surrounds and helps protect the center of the tooth and supports the enamel and cementum on the outside.

  • Inside the center of each tooth, there’s an area called the pulp chamber.

The pulp holds tiny blood vessels and nerves. The blood vessels give the pulp nutrients, and the nerves are the reason that our teeth can feel sensitivity or pain.

Inside the roots, we also find pulp in the root canals. These canals are very small tunnels that run from the pulp chamber down through the root, ending in a tiny hole in the tip of the root. Blood vessels and nerves running through the canals connect the pulp tissue to our bodies’ blood and nervous systems.

Around your roots, there’s a lot going on, too. Smaller teeth like the incisors, our front teeth, usually have only one root. Bigger teeth, like molars, usually have two or three roots. And while you’re busy biting and chewing, what’s holding these roots in place?

  • In your jaws, you have a spot where each tooth fits called a socket. But your tooth roots don’t just sit in the socket. That wouldn’t be very secure!
  • Instead, you have a periodontal ligament surrounding the roots of each tooth. Think of the ligament as groups of tissue fibers that can attach to both the cementum on your root and the bone in your jaw. This double attachment holds your teeth firmly in place. The ligament also helps cushion your teeth from the heavy pressures of chewing.
  • To make things even safer for your roots, your gums help cover them and protect them from bacteria and decay.

As you can see, your body is working hard to protect your roots. What’s the best thing you can do to keep your roots healthy? Do everything you’re already doing!

  • Keep up with your brushing and flossing, and your visits to our office. These good habits protect the teeth and gums you can see, and also the roots that you can’t see.
  • Avoid oral habits that put lots of pressure on your teeth and roots. Biting down on pencils, or ice, or nut shells, or anything not meant for regular eating means you’re risking a cracked tooth.
  • Be safe! Wear your mouthguard when you’re active.
  • Don’t wait to tell your adult if your tooth hurts. Remember, those nerves inside your teeth and roots are there to warn you if your tooth is damaged. Dr. Kerry Olszewski will be able to find out what’s wrong and have you feeling better in no time.

Even though you can’t see them, your roots are one important reason you have your beautiful smile. Taking good care of your teeth now is a way to make sure you have beautiful, healthy smiles for a lifetime!
